What to do before you arrive
University Welcome Pack
The university should send you all the documents you need before you arrive in your welcome pack, be sure you fill them in and send them back to the relevant departments. If you should need any of the forms in the welcome pack, at the bottom of this page are attached the forms you need. They are:
- Staff Unicard Application Form - Fill this form and return it, with a passport sized photo of yourself to: University Card Office, Registry Services, Room 153, Northcote House, The Queen's Drive, Exeter, Devon EX4 4QJ, or email it with a JPEG version of the photo to cardoffice@exeter.ac.uk. Your card will then be sent to the department ready for your arrival.
- PD18 Personal Details - This provides Payroll, HR and Pensions with background information. More details on the University Data Protection policy are included on the form.
- PD31 - Pre-employment Medical Questionnaire - This needs to be filled in and sent to Occupational Health Service, University of Exeter, Queen's Building (East Wing), The Queens Drive, Exeter, EX4 4QH
- Salary Payment Form - This provides the University with details of the bank account you would like to have your salary paid into. This should ideally be sent before the 15th of the month. Fill it in and return it to: Payroll Office, Exeter University, Northcote House, The Queens Drive, Exeter EX4 4QJ

Other information
In addition to the forms send to you by the University, you can fill in the IT application form which will get you an IT account set up in advance. The form is available for download at the bottom of the page. Please note that rather than sending the forms to the address on the form, you need to send it to John Rowe, by post to Dr J Rowe, Astrophysics, School of Physics, University of Exeter, Stocker Road, Exeter, EX4 4QL.
